Job Scope:
- Case management and provide counselling services to patients and their next-of-kin
- Recommend appropriate holistic care plan for patients after assessment of financial and psycho-social status of patients
- Conduct risk assessments for patients regarding their financial, emotional and health situations
- Liaise with other stakeholder to ensure effective service delivery.
- Provide administrative support such as preparing reports, tracking departmental funds and patient records
- Any other ad hoc as assigned by supervisor
Job Requirements:
- Candidates studying part-time in Social Work are welcome to apply
- Candidates with Degree/Diploma in Social Work will be considered for Medical Social Worker
